Washington State, USA, runs the largest ferry system in the US.  There
website is http://www.wsdot.wa.gov/ferries/index.cfm.  Their runs tend to go
from urban terminals to less densely populated satellite communities. They
have address some of the issues you raise, but you will probably have to
contact them for answers to your specific questions.
 
Seattle has a season "water taxi".  There is information at
http://www.seattle.gov/transportation/watertaxistudy.htm
 
Victoria and Vancouver, British Columbia, Canada also have water taxis and
aqua-bus service that might provide you with insight.
