There is a CORRECTION to these contact details.

The contact details to order the book should in fact be:  
Todd Adcock at University of Oklahoma Press. His email address is
mtadcock at ou.edu  and the fax number that I gave is now defunct.
 
An electronic order form is available which can be submitted electronically
as an attachment to Todd Adcock at the above email address or relevant
sections copied into the body of an email message.
