Department of Health (1998). Quantification of the 
Effects of Air Pollution on Health in the United 
Kingdom. Committee on the Medical Effects of Air 
Pollution. Her Majesty's Stationery Office, London.

I doubt if it will answer fully your very specific 
questions but it may help.

The British Medical Association have recently published a 
book on Health and Transport. May I suggest you search for 
their website and see if the book is there. I have yet to 
see the book but understand it covers that which you seek.
